[[Display Adult Mods]]Let's outline how the Sims 4 defines these terms, first of all. This is a straightforward run-down of how the game works.
(if: $adult1 is true)[This page assumes you have Wickedwhims installed, and shows you the gender guide with WickedWhims options included. If you do not use Wickedwhims, go turn off adult content and come back, for a rundown of how it works in the base game.]
<img src="https://i.imgur.com/QXgHbT6.png" align="center" alt="Custom Gender Settings menu">
<header>Gender</header>
<ul><li><strong>Male</strong> A male Sim uses he/him pronouns. In a few specific cases, mostly during flirty conversations, they will use different animations. These Sims have flat chests. They can equip facial hair. The sliders their bodies have access to are affected by this choice.
<li><strong>Female</strong> A female Sim uses she/her pronouns. In a few specific cases, mostly during flirty conversations, they will use different animations. These sims have round, large breasts on their chest. They cannot equip facial hair. The sliders their bodies have access to are affected by this choice.</ul>
In the sims, he/him and she/her are the only pronouns. Pronouns are hardcoded into text strings, and cannot be easily modified. (link-repeat: "There is a method to replace all pronouns in the game with them/they pronouns")[(open-url: 'https://forums.thesims.com/en_us/discussion/comment/13765977/#Comment_13765977')], but nobody has made this into an easy to install mod yet. As of writing this, other pronouns will have to be a roleplaying choice.
<header>Physical Frame:</header>
<ul><li><strong>Masculine</strong> A Sim with a masculine frame displays secondary sex characteristics sterotypically assosiated with males. With the exception of chest anatomy, which is determined by gender. They have square faces with smaller features. And narrow hips, with square buttocks.
<li><strong>Feminine</strong> A Sim with a feminine frame displays secondary sex characteristics sterotypically assosiated with females. With the exception of chest anatomy, which is determined by gender. They have soft faces with larger features. And wide hips, with a greater difference in hip/waist ratio and round buttocks.</ul>
Sims 4 sliders can be used to minimise (or exagurrate) these differences. While the two frames can never look identical, the character creator is flexable enough to create a large features/wide hips look in masculine-framed sims, and square face/narrow hips look on feminine-framed sims.
<header>Clothing Preference:</header>
<ul><li><strong>Masculine</strong> In CAS, clothes tagged with the fashion choice 'masculine' will appear to wear by default. This filter can be disabled at any time.
<li><strong>Feminine</strong> In CAS, clothes tagged with the fashion choice 'feminine' will appear to wear by default. This filter can be disabled at any time.</ul>
This is the least significant setting, as these filters can always be turned off on an outfit by outfit basis.
<header>(Pregnancy)This Sim will Be Able To:</header>
<ul><li><strong>Become Pregnant</strong> If this option is selected, your Sim can get pregnant. They can 'try for baby' with a Sim who can get others pregnant.
(if: $adult1 is true)[By default, in Wickedwhims, Sims who can get pregnant will experience a menstrual cycle. Sims fertility will be affected by their cycles. You can turn this off in the Wickedwhims settings. Setting it to "Simple Mode" will remove the menstrual cycle, and return pregnancy odds to EA's odds. (link-repeat: "Official WickedWhims documentation on pregnancy")[(open-url: 'https://wickedwhimsmod.com/sex#/sex-pregnancy/')]]
<li><strong>Get others pregnant</strong> If this option is selected, your Sim can get other Sims pregnant. They can 'try for baby' with a Sim who can become pregnant. They can also be kidnapped and impregnanted by aliens in the Get to Work expansion pack.
<li><strong>Neither</strong> These Sims cannot get pregnant, or get others pregnant. There are several reasons, in the real world, why this may be. You may also use this setting if you do not like playing with children or families.</ul>
(if: $adult1 is false)[<header>Can this Sim use the toilet standing?</header>
<ul><li><strong>Yes</strong> When the bladder meter is not in yellow-bar, a sim will stand using the toilet while peeing. If the bladder meter is yellow bar or less, a Sim will still use a toilet seated. They're pooping.
<li><strong>No</strong> A Sim will always use a toilet seated.</ul>]
(if: $adult1 is true)[<header>Does your Sim have a penis?</header>
<ul><li><strong>Yes</strong> When the bladder meter is not in yellow-bar, these sims will stand using the toilet while peeing. If the bladder meter is yellow bar or less, a Sim will still use a toilet seated. It will also affect how wickedwhims recognizes your Sims in sex interactions. A sim with a penis cannot equip a strap-on, and cannot have vaginal sex.
<li><strong>No</strong> These Sim will always use a toilet seated. It will also affect how wickedwhims recognizes your Sims in sex interactions. A sim without a penis is assumed to have a vagina. Sims with a vagina will be able to equip a strap-on, and have vaginal sex.</ul>]
(if: $adult1 is true)[All WickedWhims animation settings regarding genital choice can be changed or disabled in the Wickedwhims settings menus. You can even create an 'anything goes' game.

<header>Sim creation Tutorial</header>
Now we understand these settings, we can use them to create gender diverse Sims.
<ul><li><strong>Do research</strong> (Unless you are making a Self Sim) You should know about the gender you are trying to create. Even reading a wikipedia article can help. Look for things you can systemise into this gender system as you research.
<li><strong>There is no one right way to make a Sim</strong> There is no one combination of settings to make a cisgender, transgender, genderqueer, nonbinary, or intersex Sim. That is why I have not listed any. It all depends on each Sim. Even so, you can still be respectful when developing your Sims. If it is not a Self Sim, or a Sim with a gender and/or gender identity you are personally familier with, you should be open to criticism from members of the community you are representing.
<li><strong>Download custom content</strong> Custom Content can help expand the options you have when developing Sims. It can add more secondary sex characteristics to use to emphasise different aspects of your Sim's gender identity. See the rest of this site for recommendations and download links!</ul>
[[Behind the scenes look at the gender patch|Behind the scenes look at the gender patch]]Cheats are not game modificiations, but an in-game feature. You can open the cheat menu by holding "Ctrl + Shift + C” on your PC, or “Command + Shift + C” on Mac, all at once.

<header>General</header>
<ul><li><strong>testingcheats true</strong>
Testing cheats enables advanced cheating. This cheat may be needed to be active before certain cheats will work.
<li><strong>cas.fulleditmode</strong>
Normally, when you edit an existing Sim in Create-A-Sim, some options are locked off. This option unlocks all create-a-sim optons for all Sims. You can change an existing Sim's age, gender, traits, and more after using this cheat.</ul>
<header>Breasts Force On and Force Off</header>
Activate <strong>testingcheats true</strong> and <strong>cas.fulleditmode</strong> before using these!
<ul><li><strong>`traits.equip_trait trait_Breasts_ForceOn`</strong>
This will force the game to equip breasts (usually restricted to female Sims) on any Sim you choose. It will also give your Sim's skin the texture used for female Sims.
<li><strong>`traits.equip_trait trait_Breasts_ForceOff`</strong>
This will force the game to remove breasts (leaving your Sim with a flat chest, like the chest of male Sims) from any Sim you choose. It will also give your Sim's skin the texture used for male Sims.</ul>
You can use the former to give a male sim breasts, or the latter to give a female sim a flat chest. These cheats need to be entered during gameplay, in live mode, with the Sim you want to change as the active Sim. You then need to open your Sim up in create-a-sim (Hold Shift while you click a Sim to see this option) for the changes to become visible. You can undo this effect by repeating these steps, but using the opposite cheat. These alter the code of Sims 4, to allow new features. These are also the most liable to break in patches, or cause infinite-loading glitches when installed. I cannot promise everything linked will be working or maintained by its author in future. Some mods have lingering effects even if removed, so please read each mod carefully and consider if you really want them before installing any.

Browsing sites like ModTheSims, you may see mods with names like "Makeup equality" or "Children's clothing equality". But if these mods were released before June 2016, they were obsoleted by the gender patch, and you do not need to install them in your game. This is basic content which adds parts to create-a-sim (or build/buy mode). This stuff still occasionally breaks, but will probably never also break your game.

Many trans women will choose to tuck their genitals and hide their bulge, either to avoid transphobia or to present as more stereotypically 'feminine'. If they do show a bulge, it would be in a safe space where they don't face transphobia. (Transphobia does not exist in the Sims 4, so it would be safe and fine for women to walk around with a bulge). <header>General Hints</header>
The gender patch was released in June 2016, whereas the Sims 4 was released in late 2014. People were deleveoping CC for a while before the gender patch dropped. When downloading CC, check if it has a date on it. If it came out before June 2016, the item will only work on genders that existed before the patch.
Visit the [[Gender Guide (For Sims4Studio users)]] page if you want to use older CC sucessfully with the new patch. Or, if you want to learn how to unisex the CC you have downloaded. The terms "Male", "Female", "Masculine", and "Feminine" will be used in relation to real-world genders, but in specific to the terms used in the Sims 4. Much like how a Sim can drown in a pool if you delete the pool ladder, The Sims does not reflect the real world spectrum of genders. Using these terms is a convinience.
The Sims is, in part, a roleplaying game. A game where you take on a role of a Sim, and might take a playstyle where you make decisions based on their personality, rather than what is nessisarily the best decision for the Sim. In the Sims community, this is also known as storytelling, but roleplaying is the more generic term. In early Sims games, you were allowed to write bios for Sims to flesh them out more, and although Sims 4 no longer has this, it is still something players do in spirit. When I say something is a 'roleplaying choice', I mean its not something you cannot do in-game, but it is still a decision you can make for your Sim in how you play, think about, and/or write about them.
I did not make any of the custom content on this site and cannot offer advice or support for anything linked. I cannot even promise every link will work forever. I am not responsible for anything that happens to your game as a result of using these mods. It is up to you to read the description for each mod carefully, and comprehend it before you use it.
For the comfort of people reading this website, I will be censoring uses of outdated or fetishistic terminology with asterix. But can pretty much gurentee none of these terms are censored at the source. Please use caution while browsing.
Some sites linked may require registration before using. The Sims can be a safe enviroment to practice emotions related to sexual identity and sexual attraction. However, just because it is safe, does not mean it's //safe//. It is no substitute for real sex education, and like all pornographic material, can send harmful messages and reenforce harmful beliefs to viewers, which they may take into the real world if they do not know better.
Sex mods for the Sims //do not// add realism. They do add new interactions, but those interactions are as realistic as in any other Sims 4 mod, like the 'Become a Sorcerer' or 'Eat A Toddler' mods. Technically, you //could// eat a toddler in real life, so does the 'Eat A Toddler' mod add realism? No, because the act of doing so, and consequences for doing so will be very different in the real world than in The Sims 4. The same goes for sex mods. Furthermore, a lot of people already find the in-game Woohoo system has enough fidelity to reality for it's goofy and fun portrayal of sex.
Remember than in the real world, sex requires freely given enthusiastic consent. Use protection with all sex partners, especially with unfamilier ones, unless you have both communicated otherwise. Be honest in communicating sexual needs with a partner, and be open to them communicating their needs with you. There is more to sex than "dick juice goes splat". Queerphobia's bad. Support sex workers.

Even if you do not make CC, it is a good tool to manage your own custom content, and I recommend it for every Sims 4 player.
Every sub-page of this section will assume you have downloaded, installed, and opened up Sims 4 Studio (henceforth abbriviated as S4S) to set up the basics. You will need to tell S4S where your mods folder is, as part of this initial setup, so don't forget! Getting S4S working with Blender is optional for most tutorials here. Instructions on installation and tutorials on how to use S4S are plentiful on it's official site, and if you want to do something with S4S and do not know how, I would recommend reading the existing tutorials and/or asking on the S4S forums.

[[Behind the scenes look at the gender patch|Behind the scenes look at the gender patch]]This is my opinions section. Give it a miss if you don't care, I don't mind.
The Sims 4 gender patch was a band-aid slapped on top of a game built with a gender binary in mind. It's better than nothing, but it could have been a lot better. The fact it needs a whole guide written about it like this is kind of a bad sign for the patch, and you might get a feel for some of my problems with it when reading other pages. This is not an unbiased guide.
I think a true gender inclusive system would be something that would need to be built from the ground up. My dream (for the Sims 5 or whatever) would be for all adult Sims to use the same base body, and have all secondary sex characteristics on sliders. But this is just my personal ranting about my Sims wish list... sigh...
When googling infomation to make this mod, one of the first results was a TERF (trans exclusionary reactionary "feminist") subreddit getting all mad about it. The OP TERF was so mad, she said that she would never play the Sims 4 again. And if the gender patch took something a TERF enjoyed away from them, then good. It's a good patch.
It's better than nothing. I get sick of having to beg for scraps.
Sorry about this page.

Now we understand the gender settings, we can use them to create gender diverse Sims.
<header>General pointers: Transgender Sims.</header>
This section will focus on trans sims who fit the binary, Trans Women (mtf) and Trans Men (ftm).
Every piece of advice here is listed with the caveat that you are not making a self-sim. If you are making a self-sim, there is no problem with selecting whatever settings are the most true to yourself.
This is general advice, and you can do as you please, just be respectful about it.
<ul><li><strong>Age:</strong> Sim toddlers and children of all genders use the same frame. You technically do not have to change your Sim's gender until they reach teenagehood, and more advanced options open up, but it is up to you and/or your game's narritive. If you are playing a family and want the Sim to experience transition, teenagehood and puberty are common times for dysphoria to hit. Go to the cheats section for a guide on how to change a Sim's gender.
<li><strong>Gender:</strong> Select the gender your Sim identifies as. Like in real life, Trans Men are Men, and Trans Women are Women.
<li><strong>Physical Frame:</strong> A Trans person who can pass should have their physical frame match their gender. A Trans person who cannot pass may have a mismatched gender/frame. It is true that some Trans people are happy with an appearence that doesn't strictly pass or fit the binary. But it can also be problematic and fetishizaing to make only Trans people who cannot pass, so keep that in mind and be respectful when creating Sims.
<li><strong>Clothing Preference:</strong> This option does not matter much, but I would select the preference that matches your Sim's identified gender. It is true that sometimes, some Trans people will wear their pre-transition clothes. Some Trans people are also comfortable in any clothes. Think about your Sim's personality. But for everyday wear, it's most likely a man will just wear casual men's clothes, and a woman will just wear everyday women's clothes.
<li><strong>Pregnancy:</strong> Transitioning may give fertility a hit, but does not make a person infertile. So a Trans Man will be able to become pregnant, and a Trans Woman will be able to get others pregnant. Barring extrenuating circumstances.
A transperson who does not go through their natural puberty, has surgery to remove reproductive organs, or has bottom surgery, can have their fertility affected. (IRL, these people will often have their eggs/sperm frozen for future use.) The Sims is also a fantasy game, and IRL procedures for changing your fertility are "close", so you can sci-fi imagine an excuse for Sims to have any fertility you want! Be respectful!
<li><strong>Toilet:</strong> Anyone of any gender can pee in any way, so pick what you choose. Trans Men may choose to pee standing up, and Trans Women may choose to pee siting down, to affirm their genders, but its just piss.
</ul>
What does this have to do with gender? Absolutely nothing! I just wanted to branch out into more sjw garbage, enjoy.

I'm not a programmer or an EA team insider, so this is all theories based on my observations and speculation.
Content warning: This page heavily features unclothed sims (barbie doll anatomy), for illustration purposes.
In the beginning, when the Sims 4 was released in 2015, it had the same two binary genders as every previous Sims game, Male and Female.
A patch was released in 2016 to change this. But the Sims 4 asset creation pipeline was already well in motion, and had been set up around these two genders. And it may have been considered too expensive and/or time-consuming to completely scrap this asset pipeline and rebuilt it from scratch over a year after the game's release. So insted, and Sims 4 team coded in a system that would automatically reposition CAS item geometry, to convert items from one frame to the other. This way, items would work on all genders, and the current asset creation pipeline could continue being used.
This system has its downsides. Items don't look as clean as a manual conversion would have produced.
Not all items work with this system, and some had to be manually converted.
